Support Coordination

Our priority is to provide quality Support Coordination to our NDIS participants. We are a registered NDIS provider for Level 2 - Support Coordination.
 

SUPPORT COORDINATION

Level 1: Support Connection

​

This support is to build your ability to connect with informal, community and funded supports enabling you to get the most out of your plan and achieve your goals.  This level of service is generally provided by Local Area Coordinators (LAC’s).

 

Level 2: Coordination of Supports

​

This support will assist you to build the skills you need to understand, implement and use your plan. A support coordinator will ensure YOU receive a group of supports that will increase your capacity to live more independently and be included in your community. Your Support Coordinator will assist maintain relationships, manage day to day task also link you to the other mainstream services.

Our professional Support Coordinators can assist you in:

NDIS Knowledge – We can provide continuous coaching to improve your capacity to understand your NDIS plan, and how it is managed.

Activities - Support to you to find activities whether they are on your own, with a support worker, or joining a community group.

In-Home Care – We can assist you to find skilled support workers for meal-prep assistance, community nurses, cleaners, gardeners to assist you at home.

Finding Home - We can assist you to find the right living environment for you.

Health and Fitness - We can help you to find the right Allied health services to improve your health and wellbeing.

Study and Work – If you are interested in Study or work, we can assist you to plan ahead and make it happen.

Mainstream Services - we can also assist you to find other mainstream services.
